The Beginnings of Custom Cornea and Wavefront LASIK

If anyone is interested in how customized ablation or wavefront driven
LASIK got its start, I have just posted the first four articles in a
series of eight about the beginnings of "custom cornea" and
"wavefront LASIK".

I first learned about custom ablation at the 1998 American Academy of
Ophthalmology (AAO) meeting, but it was at the following Spring
American Society of Cataract and Refractive Surgery (ASCRS) meeting
that I came to realize that it was the future of refractive surgery.

In the previous column, I learned about using topography and other
corneal diagnostics that might lead to better LASIK surgery. At the
1999 AAO meeting, I had my first encounter with wavefront and
ray-tracing measurements, that would eventually be linked to the laser
for even better LASIK results.

At the ASCRS meeting of 2000, the first human clinical trial results
were announced. These were the first linking wavefront measurements
with the laser.

At the 2000 AAO pre-meetings (sponsored by the International Society of
Refractive Surgeons and the AAO's Refractive Surgery Interest Group),
I got my first exposure to LASEK, laser epithelial keratoplasty. In
this column, this technique is explained in detail, along with the
newest wavefront guided LASIK results (to that time).

There are four more articles/columns to go. These will be posted over
the next week or so.

I've looked at alt.lasik eyes and decided it was not a place that I
wanted to post -- too much animosity!

I read about custom lasik, it does it job in inducing less aberrations
vs. standard lasik.

QUESTION:
Based upon your considerable expertise, can you help me resolve a
dilemma, or point me to some appropriate research.
I am considering some form of custom lasik (or Intralasik) OR PRK to
resolve an astigmatism vision problem in one eye after cataract
surgery/LRI. I have dry eyes, not severe I read an info sheet from
CustomVue which says " If you currently have dry eye disease, most
eye care professionals would not consider you to be a good candidate
for LASIK." My other research seems to point toward PRK for this
reason. Prime eyedoc says not a problem. Consultant says PRK better
plan and it could still be done with some form of WaveFront.
Help please!!! Anyone!!!

Guess I should have read this first:

Anyone else?
Maybe the idea of finding a really good OD and let him/her build me a
set of glasses that will take care of the problem is rising higher up
the option list

Did you have astigmatism before the surgery? How long ago did you have
the cataract surgery? After my surgery I had some astigmatism but it
disappeared after a couple of months.

I had a a limbal relaxing incision (LRI) done to correct an existing
astigmatism, incident to cataract surgery in December. He misplaced
the LRI and the astigmatism nearly tripled. Waiting will not make it
disappear; but I am not going to have anything else done until I am
completely satisified that I have selected the proper method, and the
proper surgeon - if I choose surgery as the answer. Hence the
research.
