LASIK Research Survey

The purpose of this study to evaluate the effectiveness internet surveys in ophthalmology. Specifically, we are interested in the waning interest of Lasik and patient education. Is there a correlation?

Your participation in this research study is voluntary. You may withdraw at any time without any penalty. Your responses will be confidential and we do not require any identifying information such as your name, e-mail address, or IP address. We will do our best to keep your information confidential and anonymous. All data is stored in a password protected electronic format. The results of this study will be used for scholarly purposes only. We have no commercial interests.

This survey is for individuals who need vision correction, but have not yet gotten LASIK surgery.
You will answer a 14 question, multiple-choice survey.
If you would like to enter the drawing for a free pair of designer sunglasses, you freely give us your e-mail address, which will only be used for facilitation of the delivery should you be selected.
