Progressive Lens HELP?

I am about to get my first pair of progressive lenses. They are offering me all types of brands and materials as far as lenses at my optical shop. Can anyone recommend a brand(Hoya, Zeiss,etc.) type and material (poly,1.6,1.67,etc.) type for my prescription? From there handwriting, this is what I can make out:

+225 -450 *121/+75
+225 -400 *68/+75

Also should the frame be at least 30mm high? That seem to be the consensus.
